Five prompts for a more useful health conversation. Keep your notes privately and bring them to your appointment. This page does not collect your answers.

What matters to you right now?

Name two things you want to keep doing. That might mean hiking, lifting, traveling, intimacy, or having energy for family life.

What has changed?

Think about energy, sleep, strength, weight, hair, mood, and sexual health. When did you first notice the change? Is it persistent? You can also start while feeling well.

What else was happening?

Consider changes in sleep, work, stress, food, training, alcohol, medications, or supplements. Bring the context, without deciding the cause yourself.

What do you already know?

Bring your medication list, relevant medical and family history, and existing measurements or lab results to your clinician. Ask which checks are appropriate for you.

What would a useful next step look like?

Ask: What could explain this? What are my options? What are their benefits, risks, and costs? How will we review progress? When should I follow up?
