HOW IT WORKS

STEP 01

Baseline labs

Comprehensive 13-marker hormone panel drawn at your nearest Quest or LabCorp draw center. We issue the order; you walk in for the draw.

STEP 02

Clinician consult

60-minute video visit with a board-certified clinician. They review your labs, your symptoms, your goals — and propose a protocol.

STEP 03

Titrate and re-test

Medication ships from a licensed pharmacy. We re-test in 90 days and adjust. Most members reach steady-state in two cycles.

CLINICAL PROTOCOL

The medical brief.

Diagnosis follows the Endocrine Society 2018 guideline: persistent symptoms plus two morning fasting total testosterone draws below 264 ng/dL, or compelling clinical signs in the equivocal range with secondary marker support.

Treatment is matched to fertility goals, age, and lab pattern. Younger men with intact HPG function and fertility intent are typically started on enclomiphene or clomiphene. Men with primary or compensated hypogonadism may move directly to titrated testosterone cypionate, often with HCG to preserve testicular function.

Estradiol is monitored, not suppressed by default. Anastrozole is added only when E2 trends symptomatic, which is uncommon at well-titrated doses.

Re-testing at 90 days and 180 days. Steady-state target: free testosterone in the upper-normal range, total testosterone 600–900 ng/dL, E2 in optimal range, hematocrit and PSA stable.
